# AI-Powered Wearable Ultrasound Brings Cardiac Imaging Home

Understanding your own body is the first step to caring for it. That philosophy has guided Sai Zhou's entire career, from his early work in biomedical engineering to his current mission at CircuCare, where he's built an AI-powered wearable ultrasound device that brings cardiac imaging home in minutes instead of the 32 days most outpatients currently wait. When technology doesn't require gatekeeping, healthcare becomes accessible.

## Who am I? The question that shaped everything

Sai's questions aren't typical startup origin stories. They're foundational. Who am I? Where do I come from? Where am I going? These questions drove him to pursue understanding in the most literal way by studying the human body itself.

In his undergrad at a university in China, he started in materials science because the program ranked highly. But material science felt incomplete. It studied structures and components outside the body. What he wanted was to understand what was happening inside. By his third year, he was building biomaterials on the side, exploring how materials could work within human physiology. By the time he applied to graduate school, he accepted the offer from UC San Diego, well known for its strength in biomedical engineering, so he could study the human body directly.

That choice followed a conviction. Understanding yourself, literally understanding your own body, is how you gain agency over your health. Ultrasound became his tool for that understanding. Unlike electrical sensors that only capture signals at the skin's surface, ultrasound could see deep inside, revealing what no wearable had accessed before.

## Sonographers, the accidental gatekeepers

When Sai entered graduate research, he began exploring wearable sensors beyond the electrical ones already common in devices like the Apple Watch and Oura Ring. He studied strain sensors, mechanical sensors, and light sensors. Ultrasound stood out immediately. It was non-invasive, used no radiation, captured real-time images, and could see deep inside the body rather than just surface signals.

Ultrasound is the gold standard for cardiac imaging in hospitals and clinics. But it's also intensely operator-dependent. An ultrasound image is a 2D slice of a 3D structure. A minor shift in probe position or angle produces completely different images. Getting consistent results requires years of training. Operators need certification. They're called sonographers.

The shortage is severe. There are 25,000 unfilled sonographer positions in the US. Wait times stretch beyond a month for outpatient imaging. Patients with chest pain wait weeks to know if something is wrong. In hospitals, scans happen episodically, meaning critical cardiac events can occur between imaging sessions and go undetected.

The industry accepted this as inevitable. The technology was scarce because the expertise was scarce. The real bottleneck was the assumption that ultrasound requires human expertise to operate.

CircuCare's solution inverts that assumption. The device itself became intelligent. AI guides the device to automatically locate the heart, acquire optimal images, and interpret results. A nurse or medical assistant can place it on a patient's chest. The device does the expert work. Data streams to physicians for remote diagnosis. "You don't have to get trained to use it," Sai says. "You just apply it on your chest, and the device is going to just do the scan by itself."

## A new vision for healthcare

Sai sees healthcare shifting fundamentally in the years ahead. Hospitals will handle acute care and surgery. Monitoring, prevention, follow-up all move home instead. "Your home will be a hospital or your home will be a clinic for you," he says. Devices like CircuCare's make that possible.

A patient recovering from cardiac surgery currently stays in the hospital for days, monitored episodically. With CircuCare, they go home after two days instead of ten, conducting daily scans themselves while physicians monitor remotely. Early detection of heart failure progression happens at home, not in a clinic months later. Preventive screening becomes continuous.

The clinical validation is building. CircuCare was selected as the earliest company in the Medtech Innovator program 2025 cohort and won 1st place at the American Society of Echocardiography conference in Hawaii. Clinical trials are underway at UC San Diego Health and many other top institutions. FDA submission is planned for next year. Sai was also recognized as a Forbes 30 Under 30 honoree.

Healthcare shouldn't be something that happens to you in a waiting room. When you understand your own body, when access replaces gatekeeping, when technology serves your agency instead of limiting it, that's when control becomes possible. That's what Sai is building toward.
